Impact Health Solutions is a small team building care‑transition infrastructure for primary care. Our first product, Fireline Care Transitions, is the post‑discharge worklist clinicians actually use - risk‑sorted, SDOH‑aware, and quiet enough to live next to a real EHR without adding another login or another inbox.
The work is led by an LMSW who has spent her career on the social side of medicine - discharge planning, kinship navigation, the phone calls nobody bills for. Most "care coordination" tools are built for the metric, not for the person doing the work. We're trying to do the opposite.
We work with hospitals, FQHCs, and independent primary care practices - the teams where the social worker is also the case manager and the person who calls the pharmacy.

My operational foundation was built in demanding healthcare environments, including serving on the front lines of a Level I Trauma Center for the majority of the COVID‑19 pandemic and in various urban hospital units plagued by high utilization on the east & west coasts ever since. As a professional certified in crisis response, I understand the immense pressure these environments place on frontline workers. I am deeply committed to embedding strong clinical team support into every operational strategy, ensuring that systems protect and empower the providers running them.
True systems optimization requires looking beyond the clinical chart. It means addressing the real‑world factors that dictate patient outcomes, such as housing stability, transportation, and caregiver capacity.
This systemic approach is deeply informed by my background in economic development and community systems. Having designed municipal strategic plans, negotiated intergovernmental contracts, and managed enterprise zone initiatives, I look at healthcare through a macroeconomic lens. I understand how public policy, community infrastructure, and market data intersect to drive local economic growth and resource optimization.
