What this site collects
This marketing site (weimpacthealth.com) collects only what you submit through the "Request a pilot" form: your name, organization, role, email address, and an optional message. We use that information solely to respond to your inquiry. We do not sell or share it with third parties.
Protected Health Information (PHI)
No PHI is collected, transmitted, or stored on this marketing website. PHI is only handled inside the Fireline Care Transitions application by clinicians at partner sites, under a signed Business Associate Agreement (BAA) with the partner organization, and remains within the partner clinic's environment unless an explicit Release of Information authorizes otherwise.
Cookies and analytics
We do not currently use third‑party advertising cookies or tracking pixels on this site. Essential cookies required for the sign‑in flow (when partner clinicians log into Fireline Care Transitions) are set by our authentication provider.
Your rights
You can request that we delete information you submitted through the pilot form at any time by emailing the address below. We will confirm deletion within thirty days.
